The Community Access Program, also known as CAP includes a range of support services for the developmentally disabled individuals and families of Rutland County, Vt. It offers various residential, case management, career counseling and nursing services. The program is approved by the Commission on Accreditation of Rehabilitation Facilities, also known as CARF. The Community Access Program is administered by Community Care Network, which is a provider of community-based services in the State of Vermont. Community Care Network offers a variety of health, human, education, employment and rehabilitative services. It conducts a community rehabilitation and treatment program that features case management, outpatient treatment, psychiatric evaluation, transitional living and housing assistance services for adults. Community Care Network also provides aftercare, relapse prevention and medication-assisted therapy services through its substance abuse program.
