A day in Lehigh Valley Health Network’s Adolescent Transitions Partial Program consists of morning and afternoon groups, focusing on therapy and skill-building workshops. Our goal is to empower adolescents and their caregiving system through tools that provide ongoing support after discharge. Our collaborative multidisciplinary approach allows patients to leave the treatment experience with evidence-based strategies and skills to continue their progress. Medication assessment and management also are integral parts of treatment.
