About our DuBois Continuum of Care Community: Our community's foundation was formed in February of 1979 when the DuBois Nursing Home opened its doors as a not-for-profit organization chartered to meet the region's need for quality long-term care. In 1996, the nursing home's board of directors created DuBois Village - a personal care community - as an independent organization located on the same 13-acre campus. In 2000 and 2001, the nursing home added Memory Lane, a secure Alzheimer's/Dementia area, and Village View residential living apartments, respectively. To better serve residents' individual needs, the two entities - DuBois Nursing Home and DuBois Village - merged in 2003 and established the DuBois Continuum of Care Community, Inc. (DCCCI) With this seamless continuing of services, residents can tailor their lifestyle as needed without having to move to another community for extended care.

DuBois Village Personal Care Community offers beautiful and very comfortable rooms that are located on the first and second floors of our 3-story building. It is designed and decorated to offer a comfortable home like environment. Personal care and supervision is available 24 hours a day for those residents needing assistance with activities of daily living such as dressing, grooming, toileting, and bathing as well as assisting those with early stage dementia or other health problems that would prevent them from safely living alone. Three meals a day are provided, housekeeping and laundry services are included and staff provides medication monitoring and administration, along with daily activities designed to promote wellness and socialization. Residents can still maintain an active lifestyle and are encouraged to participate in activities and outings that match their abilities. Personal care is generally private pay with some Veteran's assistance available to those who qualify. The cost of personal care is generally less than the combined cost of living at home with home health care or nursing home care.
