History
The Stephen Klein Wellness Center (SKWC) houses Project HOME's expanded health clinic and is committed to addressing the health and wellness needs of people who are currently homeless, formerly homeless, and people living in a severely impoverished area of North Philadelphia. The mission is to reverse decades of health disparities experienced by individuals and families who are homeless or low income who struggle with mental illness and/or substance use disorder, which are often complicated by chronic medical conditions. The 28,000-square-foot wellness center provides a sorely needed health home in an area where half of the people live in poverty and almost a third in extreme poverty. It expands the health care services that Project HOME, with Jefferson Family Medicine, has been providing in North Philadelphia since 1995.

The Stephen Klein Wellness Center provides services to people of all ages, with and without health insurance. Services include: * Primary medical care * Behavioral health care * Dental care * Pharmacy * Prenatal care * Gynecology * Medication Assisted Treatment (aka Suboxone) * Phlebotomy (blood draws) * Wellness classes (nutrition/cooking, exercise, stress reduction) * Mobile nurse street outreach services * YMCA-operated fitness facility * Physical therapy at the YMCA * Legal clinic * Emergency food pantry for patients * Shower for clients/patients who are homeless * Referrals to specialty care * Assistance with applying for health insurance
