Located in Cleveland, Passport Program-Western is a Medicaid waiver program offered by the Western Reserve Area Agency on Aging. It provides information to elderly persons about long-term care services. It provides registered nurses and licensed social workers to inform older persons, family members and caregivers about various services. The program additionally offers long-term care options. It supports more than 3,000 seniors with selecting various living options. It provides services in various counties, including Cuyahoga, Geauga, Lake, Lorain and Medina in Ohio. It also offers screening and in-home geriatric assessments, adult day services, emergency response systems, meals, medical equipment, maintenance and repair services, and assistance with independent living, personal care and home modifications. Additionally, it provides counseling and transportation services to the elderly.
