Eddy SeniorCare/PACE is an integrated medical and long-term health care management program that helps chronically ill adults to live independently in their own community and avoid unnecessary nursing home placement. It offers rehabilitative, medical, social work, transportation, and personal, day and nursing care services. Eddy SeniorCare/PACE also provides a variety of health and educational programs for the community and clients and their families in New York. It is a part of The Eddy, which was founded in 1928 as a 15-plus-bed nursing home in New York. The Eddy is a comprehensive continuum of health care, supportive housing and community services that serves more than 30,000 people yearly. It is a part of Northeast Health, which is a nonprofit network that was formed in 1995 by the merger of Samaritan Hospital and The Eddy.
