HistoryHorizon Health, Inc. was created to help seniors remain living in their homes and communities as long as possible. Our four assisted living facilities focus on memory care, with one facility for gentlemen of whom most are veterans. The programs continue to grow in order to serve individuals with disabilities, transitioning to independent living, and those who need income-based rent options. Horizon Health, Inc. has added caregiver support groups and specific sessions for those with memory illnesses and their caregivers.
