At the time of its inception in 1982, Grand Valley Health Plan's (GVHP) founders laid out a blueprint for providing health care services that was uniquely different from any other provider in Michigan. The goal was a health care delivery model that focused on encouraging wellness, rather than simply treating sickness. Their vision included an integrated team approach that coordinated primary care with wellness coaching and care management. Nearly 30 years later Grand Valley Health Plan continues to set the bar in Michigan, offering healthcare services and benefits programs that are convenient, cost-efficient, and patient centered.

Grand Valley Health Plan (GVHP) is a Commercial HMO specializing in family medicine and health benefits programs. Grand Valley's staff is dedicated to the principle that the best health care begins and ends with personal, patient-centered, cost-effective, high-quality primary care. Maximizing the health of patients since 1982, Grand Valley Health Plan owns and operates community based Family Health Centers, an Urgent Care Center, two in-house Pharmacies, a Diagnostic Radiology Center, Holistic Health Center, and an OB Center in the greater Grand Rapids area. Grand Valley Family Health Centers integrate primary health care with counseling and wellness services, and is the only NCQA recognized Patient Centered Medical Home in the state of Michigan.
